Tyler ContinueCARE Hospital operates in collaboration with our host hospital, Trinity Mother Frances Hospital.
Our host hospital's mission: "As a faith based organization, it is the mission of Trinity Mother Frances Health System to enhance community health through service with compassion, excellence, and efficiency" is the foundation for our mission.



We measure our success based on the level of satisfaction expressed by our patients, physicians, our host hospital, our staff, payors, and regulators.
We lease space from our host hospital. In addition we purchase certain ancillary services from our host hospital.
