A Long Term Acute Care Hospital (LTAC) patient stay
most likely would occur after a short term acute
care hospital stay. Many patients are then able
to transition from the LTAC to acute rehab, skilled
nursing and/or go home. Hospice care is also a post
acute treatment option for patients with a terminal
disease who have a life expectancy of six months
or less.
Tyler ContinueCARE
Hospital, as
a Long Term Acute Care Hospital, is dedicated to
providing quality care by supporting the development
of a continuum of care between the acute phase and
long term acute phase of a patient's illness.
Our innovative and distinctive physician driven
approach allows us to bring new meaning to the "hospital
within a hospital" model. As a separate, fully
licensed, and staffed hospital we are able to provide
intensive and complex medical treatment to patients
requiring longer lengths of stay for acute care.
We have specialty programs which are designed to
meet the needs of long term, acutely ill patients
with conditions ranging from ventilator dependence,
respiratory failure, chronic pulmonary problems,
complex and severe wounds, infections requiring
long term antibiotic therapy, to support with physical,
occupational, and nutritional therapies, as well
as pain control.
