Our Mission...
As a "hospital with-in a hospital", it is the mission of
Tyler ContinueCARE Hospital to enhance community health through service with COMPASSION, CARING, ACCOUNTABILITY, RESPECT and EMPATHY.

Long Term Acute Care Hospitals have become essential to an effective continuum of care in our health care system. To meet this need Community Health Corporation formed a subsidiary ContinueCare Corporation to develop long term acute care hospitals.

Community Health Corporation was created in 1996 by member organizations of VHA Southwest, a cooperative of not-for-profit hospitals in Texas and New Mexico, and supported by working capital from a number of VHA Southwest health care organizations.

CHC was formed to preserve the not-for-profit status of community-based health care organizations, which is important to a community hospital because it defines the central mission of the hospital as offering quality health care services to all community residents in a cost-effective manner. CHC also strives to help community-based health care organizations improve operations and care through partnership and shared vision.
Tyler ContinueCARE Hospital is a 51 bed LTAC hospital located on the 4th floor of our host hospital Trinity Mother Frances Hospital. We enjoy the best of both worlds by being in partnership with a large 352 bed acute care hospital yet maintaining our small hospital atmosphere. Our employees are a dedicated team who work together to deliver the best possible care for our patients. Our longer length of stay allows our staff the opportunity to get to know their patients and families and spend quality time making a difference.

We are proud to be a part of the medical community in this beautiful East Texas setting. Tyler is nestled in the piney woods of East Texas, surrounded by the beauty of nature with many lakes and golf courses. This area is an ideal spot to work and live. The mild climate provides for year round abundant outdoor activities such as: tennis, golf, biking, fishing, boating, and swimming just to mention a few!



Hospitalist Program

Tyler ContinueCARE Hospital works with a dedicated group of hospitalist who are devoted to efficiently managing the process of inpatient care from the time of admission to discharge from our hospital. Hospitalist can be internal medicine or family medicine physicians and have training in specialties such as, cardiology and intensive care. These physicians assume the primary responsibility for managing the medical needs of the patients who are admitted to our hospital.


A hospitalist does not replace the patient's primary care physician, surgeon, or other specialist, but works in close consultation with them to provide total case management during the patient's entire stay in our hospital. The hospitalists keep the patient's primary care physician up-to-date on their care and treatment plan.

Our hospitalists are committed to providing quality and compassionate care to our patients and their families. Whether explaining a medical procedure, updating the family on their loved one's condition or planning appropriate care after discharge, our hospitalist are dedicated to ensuring that patients are comfortable and receive very good care during their stay in Tyler ContinueCARE Hospital.

Hospitalists do not see patients outside of the hospital setting. While a patient is under the care of a hospitalist, the primary care physician receives continuous updates about the patient's condition and plays a major role in the treatment plan.

Once a patient is discharged from our hospital, any follow-up appointments should be made with the primary care physician who has consulted with the hospitalist in developing the treatment plan.
